This article will describe how to access a particular contact’s responses to a Contact Demographic-related form. Contact Demographic forms can be used to pull information and further segment your contacts.
Finding a Contact Profile
1. Login to your Member365 Administrator Dashboard.
2. Type the name of the contact whose form you are attempting to retrieve into the “Search Contacts” bar at the top, and click their name when it appears in a drop-down box.

3. On the contact profile, find “Contact Demographics” in the lower-right corner.
4. In the “Contact Demographics” box (that lists all possible contact demographics), if the box beside a demographic is marked with a blue checkmark, this means that demographic form is applicable to the contact. Beside the check box, there will either be an eye symbol or a circle with a cross through it. If it's an eye, then the contact can see the form in their Member Portal, if it's the crossed circle, they cannot. To view the form (and how it's filled out for this particular contact, simply click on the name.
